GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

Under the general direction of the Lead Case Manager the crisis case manager will be responsible for providing mobile crisis outreach services such as crisis response, crisis assessment, crisis screenings, assistance with inpatient hospitalization, and in-home support services on a rotating schedule.  Coordinates, links, and provides follow up intervention services to persons in crisis.  Services will be provided in a variety of settings to include home, community, school, jails, hospitals, etc.  Must be able to physically respond to activated crisis calls within one (1) hour regardless of designated level of intensity, urgent ‘vs’ emergent.  This position requires extensive local travel, on-call responsibility, the ability to work changing shifts to include evenings, some weekend coverage, have rotating days off, and be available to work overtime.  Cellular phone, laptop, and hotspot availability is required.

MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:

Bachelor’s degree from an accredited college or university, with a major in social, behavioral, or human services, plus two (2) years of full-time work experience in human services case management.  Must meet Qualified Mental Health Professional (QMHP) requirements.
Applicant must live within one hour from the Dimmit/Zavala/Maverick service area due to crisis response time requirements.
Demonstrate ability to read, write and speak English and Spanish.

PREFERRED JOB QUALIFICATIONS:

Prior working experience with a community mental health center.
Prior experience providing LCDC, crisis intervention services and on-call services in a behavioral health setting.
Prior knowledge and experience with trauma-informed clinical interventions for mental and emotional wellbeing.

KNOWLEDGE, SKILLS, AND ABILITIES:

Ability to work independently, to organize work efficiently, and to communicate effectively both verbally and in writing.
Knowledge of individual needs/characteristics associated with mental illness.
Ability to develop productive relationship with individuals and family members that focus on individual specific outcomes.
Extensive skill in individual needs assessment and the ability to respond to the changing needs of the individual and the ability to work in a crisis.
Ability to work cooperatively and effectively with other professionals, individuals and groups, exercise tact, initiative, and good judgment when dealing with people, prepare reports and recommendations for administrative use.
Knowledge of local community resources, how to access those services and use them effectively.
Knowledge of the principles, methods, techniques and practices of social work and chemical dependency, the functioning of mental health hospitals and/or out-patient mental health clinics and schools.
Ability to relate to clients with a variety of age, sex, economic, educational, and professional background.
Ability to apply and integrate theory, knowledge, values, and practices of social work and/or counseling profession.
Ability to engage in problem solving process.
Ability to understand, articulate and apply the dynamics of chemical abuse and dependency.
Demonstrate skills in the use of personal computer to record client data, written documentation of client assessments and interventions, correspondence, or reports.
Working knowledge of the DSM-IV.
