GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Community Care Coordinator (CCCs) are relied upon to deliver the full range of services, integrated physical health and behavioral health services for CRCS consumers.  CCCs are responsible for coordinating the access to medical, dental, and other allied health professional care for the physical health needs of CRCS consumers.  CCCs coordinate consumers’ physical health care with the entire CRCS team of behavioral health staff .  CRCS priority populations served include adults with severe, chronic mental illness and their families to include those with dual disordered mental illness and substance abuse disorders, as well as children, and adolescents, with qualifying dental/emotional conditions.  CCCs collaborate with medical, dental, and allied health agencies, clinics, and private providers.  CCCs coordinate, facilitate, track, broker, and document consumer/patient referrals and appointments for medical services such as medical history and physical exams.  Medications/prescriptions, injections, laboratory tests, and any other medical service required or directed by physician orders.  CCCs document work daily utilizing agency approved software.  Clinical supervision will be provided by appropriate staff.  Administratively reports directly to Integrated Program Behavioral Health Consultant within the overall CRCS system.  CCCs constantly exercise vigilance in maintaining confidential files and reports and contribute to the reports due to HHSC on grant achievement.  Position requires extensive travel, often outside the typical work hours.  Must be willing to transport consumers using an agency vehicle.  Required to adhere to the dress code and agency policies of the Atascosa Health Center in Tilden where the position is located.  CRCS is a trauma-informed care agency, and all staff must be trained in Trauma Informed Care.  This is a grant-funded position.  May be required to transport individuals to appointments.

MINIMUM JOB QUALIFICATIONS:
Applicants must meet the following minimum qualifications to be eligible for the position:
 
  • Must have a high school diploma or equivalent GED.  Must have at least 2 years of experience working in a community setting or social service environment setting.
OR
 
  • Applicants must meet the minimum qualifications below to be eligible for an HCN position.  Academic degrees must be from a regionally accredited college or university.  In general, people majoring in Nursing or in a behavioral, social, or  human services field are encouraged to apply.  Texas Certified Community Health Workers.  Must meet QMHP requirements as follows:
  • Bachelor’s degree from an accredited college or university with a major in social, behavioral, or human service field (psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • One year of work experience as a case manager or caseworker.

PREFERRED JOB QUALIFICATIONS:
 
  • Bilingual English/Spanish.
  • Associates degree in social services.
  • One (1) year work experience in a health care/clinic environment.
  • Three (3) years of experience in a community health setting.
  • Texas Certified Community Care Coordinators, Promator/a, Certified Nurse Assistant (CNA) and Certified Health Unit Coordinators (CHUC) are also encouraged to apply.
  • One (1) year of experience working with individuals with mental illness and/or dual diagnosis in a community setting.
  • Resident of McMullen County.

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of individual needs/characteristics associated with mental illness.
  • Knowledge of community-based behavioral health services.
  • Ability to work in a fast-paced health services work setting and to meet deadlines.
  • Ability to problem solve in a creative, viable and realistic manner.
  • Ability to support Integrated Care program to improve health outcomes of residents in Atascosa County.
  • Ability to make appropriate referrals and linkages and to overcome barriers between service systems.
  • Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
  • Knowledge of local community resources, how to access those services and use them effectively.
  • Knowledge of the principles, methods, techniques and practices of psychology, behavioral science, social work, chemical dependency, and the functioning of mental health hospitals and clinics.
  • Skills for public service referrals/contacts on the telephone and in person.
  • Skills for service delivery and case management.
  • Skills with needs assessments, treatment planning and responding to individual’s changing needs.
  • Skill with contributing to a treatment team of physicians, nurses, counselors, and others.
  • Skills for independent, organized, and efficient work and effective verbal and written communication.
