GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Provides mental health case management functions as outlined by Texas Administrative Code, Title 25, Part II Service Coordination Chapter 412, Subchapter 1 to a designated caseload of TCOOMMI individuals that have been referred from the Texas Corrections Department, Huntsville, office (TCOOMMI) as well as local Adult Probation & Parole Department, which includes assessment, treatment planning, monitoring of services and crisis prevention and management.  Utilization of TX Resiliency and Recovery Management procedures in assessing and providing treatment to individuals identified with mental health needs.  Develops and referral resources for service provision allowing for consumer choice where available of providers.  Complies with Texas Correctional Office on offenders with mental/medical impairments, contract requirements and follows TCOOMMI Program Guidelines and procedures in providing, accessing, or referral to necessary treatment services.  Overall job duties to provide services for offenders in the TCOOMMI special needs offenders program, who are on probation or parole who live in Atascosa/Wilson County and who meet TXDSHS priority population definitions.  Authorizes provision of services based upon assessed needs and availability.  Ensures the best value of resource allocation to service provision.  TCOOMMI Case Manager is to provide intensive mental health services to offenders with severe and persistent mental illness.  The focus of services is to stabilize symptoms, reduce the risk of recidivism, increase awareness of and participation with community and natural supports, develop skills in self-advocacy, and gain the ability to participate in independent  mental health care.  Services focus is on the reduction of intensity and frequency of symptoms, improvement in core community reintegration skills, increase in self-reliance, independent living skills, and effective peer, community, and family interactions.  As well as participating in community coalition meetings, training, and criminal justice training seminars as designated to maintain required CEUs per the TCOOMMI Performance Contract.  Position reports to the TCOOMMI Director.

MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:
  • Bachelor’s degree from an accredited college or university with major courses in social, behavioral, or human services plus a minimum of 3 years of documented work experience in behavioral health services.
  • Must maintain a valid drivers’ license and automobile insurance coverage.
  • Must be able to pass required background check(s) and pass random drug screening(s).

PREFERRED JOB QUALIFICATIONS:
 
  • Demonstrate ability to read, write and speak English and Spanish.
  • Prefer a minimum of 2 years’ experience in a like or similar position working with parolees.
  • Preference will be given to an individual that has training and experience with the ANSA/CANS HHSC Assessment protocols.
  • Training in trauma informed care through class completion or other formal training.

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of the principles, methods, techniques, and practices of social work.
  • Skill in interviewing and assessing treatment needs of persons served.
  • Ability to communicate effectively both written and oral.
  • Good organizational skills and the ability to plan and implement consumer services.
  • May require abilities to supervise the work of others, to interpret program methods and procedures, and to provide guidance to other staff.
  • Ability to work under conditions requiring physical stamina and the ability to work in a stressful environment.
  • Ability to work in a rapid paced environment in an organized and professional manner.
  • Working knowledge of mental health issues, services, and treatment as well as knowledge of the criminal justice system.
  • Ability to self-monitor production and quality with minimal assistance.
  • Ability to work as a team member in a diverse environment interfacing with both internal and external customers.
