Agency vehicles may be available; however, mileage is reimbursed for some activities requiring use of personal vehicles.  The selected applicant must be able to attend approximately five working days of staff development orientation in Lytle, TX.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:


Camino Real Community Services is implementing a specialized program focused on First episode psychosis.  The Coordinated Specialty Care (CSC) program will work with individuals who are between the ages of 15-30 years of age and are in the initial stages of a primary psychotic disorder.  CRCS is looking for qualified, and motivated team members who are interested in rendering specialized services.

The CSC case manager under the supervision of the CSC Team Leader, will work with individuals and families of adolescents and young adults with severe emotional disturbance (SED) or severe and persistent mental illness (SPM) using the RAISE model of care.  Facilitating a team planning process that is strengths based, focused on underlying needs, and determined by the individuals and families of those enrolled.  The focus of this position is providing intensive services to a caseload of up to 30 individuals and their families enrolled in the CSC program, adhering to agency productivity standards.  The CSC case manager is also responsible for developing and organizing stakeholder meetings and participating in outreach efforts.  A flexible schedule is required to meet the needs of the enrollees served.  The CSC case manager needs a good working knowledge of available community resources and must be able to effectively build working relationships with those resources.  Extensive travel is required.  Examples of daily activities would include working in a team environment, coordinating entry into the program, providing education around understanding diagnosis, benefits of medication, and finding mental health recovery.

Administratively reports directly to the CSC Team Leader.  Camino Real Community Services is a trauma-Informed Care Agency and implements the principles of trauma informed care.

MINIMUM JOB QUALIFICATIONS:
 
  • Bachelor’s degree from an accredited college or university, with a major (or the equivalent to a major) in psychology, social work, medicine, nursing rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention, or a registered nurse, plus a minimum of 1 year of documented work experience in behavioral health services or in a community mental health center.

PREFERRED JOB QUALIFICATIONS:
 
  • Demonstrated ability to read, write and speak English and Spanish.
  • Prefer a minimum of 2 years’ experience in a like or similar position.
  • Preference will be given to an individual that has training and experience with the ANSA/CANS HHSC Assessment protocols.
  • Training in trauma informed care through class completion or other formal training.
  • Experience with working with individuals who experience persistent symptoms of psychosis.

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of individual needs/characteristics associated with severe and persistent mental illness.
  • Knowledge of community-based service delivery system and the role of case management.
  • Knowledge of local community resources, how to access those services and use them effectively.
  • Knowledge of the principles, methods, techniques and practices of psychology, behavioral science, social work, chemical dependency, and the functioning of mental health hospitals and clinics.
  • Skills with needs assessments, treatment planning and responding to consumer’s changing needs.
  • Skill with contributing to a treatment team of physicians, nurses, counselors, and others.
  • Skills for independent, organized, and efficient work and effective verbal and written communication.
  • Skill for documenting work done as well as drafting reports and recommendation for administrative use.
  • Ability to work cooperatively and effectively with other professionals, individuals, and groups.
  • Ability to exercise tact, show initiative, and use good judgment in dealing with people even in crisis.
  • Ability to develop productive and outcome focused relationships with individuals and family members.
  • Ability to recognize and overcome interagency obstacles for accessing services for individuals.
  • Ability to identify problems/gaps in service.
  • Ability to advocate effectively for an individual in the target population.
  • Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
