Agency vehicles may be available; however, mileage is reimbursed for some activities requiring use of personal vehicles.  The selected applicant must be able to attend approximately five working days of staff development orientation in Lytle, TX.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:


The CSC case manager, under the supervision of the CSC Team Leader, will work with individuals and families of adolescents and young adults with severe emotional disturbance (SED) or severe and persistent mental illness (SPMI) using the RA1SE model of care.  Facilitating a team planning process that is strengths based, focused on underlying needs, and determined by the individuals and families of those enrolled.  The main focus of this position is providing Intensive Services to a caseload of up to twelve individuals and their families enrolled in the CSC program, adhering to agency productivity standards.  The CSC Case Manager is also responsible for developing and organizing stakeholder meetings and participating in outreach efforts.  A flexible schedule is required to meet the needs of the enrollees served.  The CSC Case Manager needs a good working knowledge of available community resources and must be able to effectively build working relationships with those resources.  Extensive travel is required.  CRCS is a trauma informed system of care and implements the principles of trauma informed care.

Examples of daily activities would include working in a team environment, coordinating entry into the program, providing education around understanding diagnosis, benefits of medication, and finding mental health recovery.   

MINIMUM JOB QUALIFICATIONS:
 
  • Bachelor’s degree in Social, Behavioral, or Human Services plus two (2) years of work experience with children.
  • Must meet Qualified Mental Health Professional (QMHP) qualifications.
  • Must possess a valid driver’s license with an acceptable driving record.  
  • Must have good computer skills.
  • Must pass a background check.

PREFERRED JOB QUALIFICATIONS:
 
  • Related Master’s Degree and professional licensure (i.e., LPC, LPC-A, LCSW)
  • 3-5 years’ experience in a community mental health setting
  • Bilingual in English and Spanish

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of mental health and IDD, community service delivery, and the needs of consumers who have major mental illness and concomitant behavioral disorders, i.e., chemical dependency, personality disorders.
  • Knowledge of the principles, methods, techniques, and practices of social work. 
  • Skill in interviewing and assessing treatment needs of persons served.
  • Ability to communicate effectively both written and oral.
  • Good organizational skills and the ability to plan and implement consumer services.  
  • May require abilities to supervise the work of others; to interpret program methods and procedures; and to provide guidance to other staff.
  • Ability to work under conditions requiring physical stamina and the ability to work in a stressful environment.
