GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:


Under the direction of the designated Innovative Healing Centered Project (ICHP) Team Leader, the IHCP Case Manager increases continuity of care by managing relationships with tertiary care providers, transitions-in-care, and following referrals to other allied health professional care for the physical health needs of CRCS consumers.
 
  • Coordinate physical health care with the entire CRCS team of behavioral health staff, promote timely access to appropriate care, increase utilization of preventative care, reduce emergency room utilization and hospital readmissions, and increase health comprehension through culturally and linguistically appropriate education, case management, and rehab skills.
  • Interagency collaboration is essential to this position especially with connecting individual patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
  • Responsible for coordinating physical healthcare and wellness plans for consumers, arranging, and tracking appointments, educating clients/patients about health matters, and coordinating services for other aspects of CRCS consumers’  wellbeing.
  • Follow-up with patients, families, caregiver(s), providers, and community resources to ensure referrals and appointments are scheduled and are synchronized with the physician and nursing staff, to include obtaining, managing, and tracking annual physical health exams in electronic health record.
  • Coordinate, facilitate, track, broker, and document consumer/patient referrals and appointments for medical services such as medical history and physical exams, medications/prescriptions, injections, laboratory tests, and any other medical services required or directed by physician orders.
  • Document indicators of consumers’ health status and may perform other office duties with priority being tasks in support of services focused on wellness promotion, disease prevention, and intervention with chronic health conditions/concerns.
  • The job requires local travel for community-based service provision and also some flexibility in work hours and schedule.  Clinical supervision will be provided by appropriate staff.  Administratively, reports directly to IHCP Team Leader and is guided and mentored by senior CRCS staff.
MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:
 
  • Bachelor’s Degree from an accredited college or university, with a major in (or the equivalent to a major), in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention; plus 3 years paid work experience performing case work or case management duties.
OR
 
  • Master’s degree from an accredited college or university in above note fields.
  • Must possess good computer skills.
  • Must be able to use personal vehicle for travel to provide job duties when an agency vehicle is unavailable.

PREFERRED JOB QUALIFICATIONS:
 
  • Demonstrated ability to read, write, and speak English and Spanish.
  • One (1) year of work experience in case management or field related to the duties of the position.
  • One (1) year of experience working with individuals with mental illness and/or dual diagnosis in a community setting.
  • Familiarity with psychiatric and mental health related terminology to include crisis response.
  • Familiarity with Electronic Medical Record (EMR).

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of individual needs/characteristics associated with severe and persistent mental illness.
  • Knowledge of community-based service delivery system and the role of case management.
  • Knowledge of local community resources, how to access those services and use them effectively.
  • Knowledge of the principles,  methods, techniques and practices of psychology, behavioral science, social work, chemical dependency, and the functioning of mental health hospitals and clinics.
  • Skills with needs assessments, treatment planning and responding to consumer’s changing needs.
  • Skill with contributing to a treatment team of physicians, nurses, counselors, and others.
  • Skills for independent, organized, and efficient work and effective verbal and written communication.
  • Skill for documenting work done as well as drafting reports and recommendation for administrative use.
  • Ability to work cooperatively and effectively with other professionals, individuals, and groups.
  • Ability to exercise tact, show initiative, and use good judgment in dealing with people even in crisis.
  • Ability to develop productive and outcome focused relationships with individuals and family members.
  • Ability to recognize and overcome interagency obstacles for accessing services for individuals.
  • Ability to identify problems/gaps in service.
  • Ability to advocate effectively for an individual in the target population.
